Free Athletic Physicals, May 8
As the school year comes to a close, it’s time to complete our sports medicine requirements for the upcoming school year. This includes athletic physicals, physical history, permission to treat, and HIPPA forms. These items are required for all students entering grades 7-12 who plan to participate on any athletic teams during the 2018-2019 school year.
This year we have partnered with Martin Orthopedics, Dr. Joel Smith, and Ms. Maddie Awtrey to offer the required physicals on campus at no charge to all enrolled students. The physicals are scheduled to take place on May 8 at 6:30 p.m.
We will release more information regarding logistics and the required forms as the date approaches. I want to strongly encourage all athletes to take advantage of this opportunity and complete the required paperwork. This will save Ms. Awtrey, our coaches and parents valuable time in the future. Again, athletes who do not complete a physical and the corresponding paperwork will not be allowed to participate.
There is no cost or charge to your insurance, so if you choose, it will allow you to use your yearly free physical with your pediatrician or family doctor. To provide this service to students free of charge, I am asking any parents that are qualified to take basic vitals (blood pressure, pulse, and vision) to volunteer one hour of your time to assist.
